Salesforce adds care coordination features to health cloud

Salesforce, a customer relationship management services provider, launched two new features on its Salesforce Health Cloud: Health Cloud Empower and Concurrent Care Plans.

Health Cloud Empower enables healthcare providers — including those outside a physician’s office, such as home nurses, hospice care and family members — to communicate with one another through centralized care team lists. The feature also offers a centralized platform for providers to assign care plans to patients.

Concurrent Care Plans facilitates collaboration between healthcare professionals caring for patients with multiple medical issues. The feature enables different healthcare teams with disparate workflows to coordinate care by assigning multiple care plans in a centralized location. The platform also allows providers to assign permissions on certain information to control who has access to patient data.

“Health Cloud Empower and Concurrent Health Plans enables patients and providers to manage their care journeys from anywhere,” said Joshua Newman, MD, CMO of Salesforce. “With most care today happening outside of the hospital or doctor’s office, engaging patients from any device is paramount.”
