ProHealth Care First to Use Epic's Population Management Tool

Waukehsa, Wis.-based ProHealth Care has become the first healthcare system to use Epic's Cogito data warehouse to manage population health.

The warehouse is being used to create physician profiles, a physician-friendly interface that allows providers to see which patients are in need of screenings or treatment. Profiles are provided for the 460 physicians in the ProHealth Solutions accountable care organization, which includes both employed and affiliated physicians.

The profiles are based on 54 quality metrics such as cancer screenings and cholesterol screenings that match the goals of the Medicare Shared Savings Program, in which ProHealth Solutions is a participating provider. The interface is color-coded by metric, showing a physician where national metrics are not being met and providing a list of patients in need of a care intervention in that metric.

"A typical primary care physician may have more than 2,500 patients, so without tools like this, it’s almost impossible to track each patient and know who is behind on care," said Steve Kulick, MD, CMO of ProHealth Care, in a news release. "The profiles allow us to easily identify patients who are in need of screenings or treatment."
