The North Carolina Healthcare Association implemented Clarify Health Solutions' advanced analytics platform.
The Care Prism analytics platform features an extensive data set, linking clinical claims and social determinants of health insights. The association plans to use the technology to support its Pneumonia Knockout Campaign, which has a two-year goal to save 1,000 lives.
Clarify Health Solutions' platform gives clinicians involved in the campaign insights into what happens after patients leave or are discharged from healthcare organizations. Ninety-plus hospitals are involved in the Pneumonia Knockout Campaign.
"This partnership has allowed us to look beyond the hospital and examine care transition patterns specific to our high-risk pneumonia patients," said Karen Southard, RN, the association's vice president of quality and clinical performance improvement. "The additional information has been very helpful to us and our members in developing targeted care coordination interventions to improve outcomes."
