Michigan HIE partners with Holon Solutions

Great Lakes Health Connect, Michigan’s health information exchange, joined forces with health IT firm Holon Solutions to enable improved management of the state’s Medicaid population.

The HIE will use Holon’s CollaborNet technology to integrate clinical and non-clinical data, including social determinants of health. The technology will present the information via alerts. Additionally, the technology will enable providers to refer patients to community organizations that can assist them with a myriad of social issues, including transportation.

The HIE includes a network of 129 hospitals and more than 4,000 primary, specialty and allied care organizations, as well as nearly nine million unique patient records. It is joining forces with Holon in part to meet the goals of Michigan’s State Innovation Model, which lays down standards for provider management of Medicaid populations.
