The solution suite helps healthcare organizations meet the challenges of the hierarchical condition category model as it is designed to capture the disease burden and risk-adjustment factor scores. The suite features front-end physician HCC support, back-end HCC audit and workflow management and HCC assessment and planning.

“Maintaining quality documentation for the HCC framework is complex and requires a multi-pronged strategy that continually evaluates, monitors and improves documentation quality. By improving the specificity and quality of documentation in both real time and retrospectively, our HCC solutions help healthcare organizations holistically enhance their risk-adjustment performance: optimize ROI, identify gaps in patient care and provide enterprise-wide insights into risk-adjustment documentation,” said Scott MacKenzie, CEO of M*Modal, in a statement.
