Lubbock Cardiology Clinic posted a letter online notifying patients their medical records were accessed by an unauthorized individual from Dec. 15, 2013 to Jan. 30, according to a Lubbock Avalanche-Journal report.
The unauthorized user accessed and downloaded patients’ names, addresses, phone numbers and Social Security numbers, according to the report.
