Ridgewood, N.J.-based Valley Health System is launching a hospital at home program this summer to deliver high-acuity inpatient care directly to eligible patients in their homes.
The program — developed in partnership with Minneapolis-based Inbound Health — aims to enhance patient experience by delivering hospital-level care in the comfort of patients’ homes, the health system announced in a June 23 news release.
Valley Health System’s program will treat patients with conditions such as pneumonia, congestive heart failure and cellulitis. Before enrollment, patients will be evaluated at The Valley Hospital to determine medical eligibility and whether their home environment is suitable for care.
Once enrolled, patients will receive daily clinical assessments, 24/7 remote monitoring, medical supplies and in-home visits from nurses and clinicians, according to the release.
Inbound Health will provide virtual and in-person clinical support, logistics, technology and care protocols to operate the program.
