Hartford HealthCare, Pennant to form unified home care operating entity

Hartford (Conn.) HealthCare and The Pennant Group are deepening their home-based care partnership in Connecticut and plan to transition the collaboration into a unified operating entity.

The move builds on a partnership launched in May 2024 between Hartford HealthCare at Home and Pennant, the parent company of affiliated home health, hospice, home care and senior living companies.

According to a June 4 news release, the organizations are strengthening their collaboration through mutual investment in home-based care services in Connecticut as they work toward creating a unified operating entity.

The organizations said the collaboration has helped Hartford HealthCare at Home earn its first 4-star rating from CMS.
