The Medicare Innovations Collaborative is an initiative to implement a portfolio of geriatric service models into hospitals and healthcare systems. The program began in late 2009 with the following participants: Milwaukee-based Aurora Health Care, Charlotte, N.C.-based Carolinas HealthCare System, Crouse Hospital in Syracuse, N.Y., Danville, Pa.-based Geisinger Health System, Allentown, Pa.-based Lehigh Valley Health Network and University Hospitals Case Medical Center in Cleveland.
The organizations implemented several programs to improve geriatric care, including a model for care transitions and a palliative care service. Members of the collaborative said the perceived prestige of participation in the collaborative helped drive change among their leaders and was one of the top two reasons for success, according to the study.
