Innovaccer, a Silicon Valley-based healthcare analytics company, unveiled a free tool to help providers identify and monitor the impact of social determinants of health.
Here are three things to know.
1. The Social Determinants of Health Portal includes patient population data from the past six years across more than 3,000 U.S. counties.
2. The tool presents data on a nationwide, state and county-level scale through color-coded line charts, heat maps and bar graphs.
3. Abhinav Shashank, co-founder and CEO of Innovaccer, said physicians can use these insights to connect patients with appropriate community resources to address their social circumstances.
