In-home care provider links clinicians, community services network

An in-home care services provider is introducing software that will connect its 4,000 clinicians to community organizations addressing social determinants of health.

Signify Health acquired technology platform TAVHealth in March and has integrated the platform with its offerings to create Signify Community, a network of organizations that can help patients address social issues affecting their health, such as housing and transportation.

"Through our house call programs we've long observed and reported on the social stresses that drive adverse outcomes in our patients," Kyle Armbrester, CEO of Signify Health, said in a news release. "Now we'll be able to swiftly act on these complex issues and build healthier communities while we improve the lives of individual patients."
