The ACO teamed up with ClosedLoop.ai to introduce the AI-based predictive model. The AI analyzes patient demographics, social determinants of health, claims and clinical activity to determine which patients need the most outreach.
“We want to identify what we refer to as the ‘socially isolated’ or people without nearby friends or family so our care teams can proactively educate and offer assistance to people in regards to COVID-19,” said Art Jones, MD, CMO at Medical Home Network.
The ACO includes 10 federally qualitied health centers, three hospitals and serves approximately 120,000 Medicaid patients in the Cook County, Ill., area.
