Humana’s care management program will use tools from Happify Health to screen chronic heart condition patients for signs of depression or anxiety. Those who qualify will be enrolled into the Happify Health digital intervention, which provides targeted emotional support for these patients.

The Happify Health program aims to decrease rates of mental illness, since heart conditions may worsen for patients whose depression goes unmanaged.
