HIMSS to Debut Continuity of Care Model at HIMSS14

At its annual conference next month, HIMSS will add another maturity model to help healthcare providers gauge how effectively they are using health IT to improve care delivery.

The Continuity of Care Maturity Model will highlight the importance of interoperability, information exchange, care coordination, patient engagement and analytics in providing both complete individual and population healthcare. The model will evaluate providers’ use of IT, data analytics and patient engagement in improving outcomes.

The seven-stage model is designed to compliment HIMSS’ other maturity models, such as the Electronic Medical Record Adoption Model.
