HealthFusion introduces chronic care EHR tool in response to CMS’ chronic care incentive

The makers of the cloud-based MediTouch EHR, HealthFusion, announced the launch of a chronic care management module in response to CMS’ Chronic Care Management changes.

CMS’ Chronic Care Management changes, which went into effect Jan. 1, 2015, changed the billing codes to allow physicians to charge patients with two or more chronic conditions for 20 minutes of chronic care management per 30 day period. Physicians must provide patients with a written agreement and a copy of a care plan, and the patient must be classified as a CCM patient in the physician’s EMR.

The EHR’s new Chronic Care Management program is an instruction module that includes all the details and instructions of the new Chronic Care Management Benefit program, making it easier to implement the additional billing and notification process into the practice. It is included for free in HealthFusion’s EHR with an infographic, a white paper that outlines the requirements and an example of a patient agreement.
