The company offers a care coordination platform that stores patient information as well as provider instructions. It also provides tools to plan for appointments and manage tasks. Patients have access to their records online and via mobile apps.
CareSync has undergone numerous financing rounds, raising approximately $18 million at the end of 2015, according to the report.
