The new platform is designed to improve care coordination between all areas of patient care and discharge; establish a common patient progression plan and length-of-stay target; improve bed placement; and eliminate patient care barriers.
Grady Health System is one of the largest safety net health systems in the country. It includes the 953-bed, Atlanta-based Grady Memorial Hospital, six neighborhood health centers, Crestview Health & Rehabilitation Center as well as Children’s Healthcare of Atlanta at Hughes Spalding, an affiliate of Children’s Healthcare of Atlanta.
