The program uses remote patient monitoring software from Telemetrix combined with coaching provided by remote care management company Remote Care Partners, according to a May 11 press release from Telemetrix.
The program is also integrated into the health system’s provider workflow.
Piedmont Healthcare says it plans to roll out the program at additional facilities.
