Florida health plan mailing error causes data breach

CarePlus Health Plans in Doral, Fla., notified approximately 1,400 members that some sensitive information may have been mistakenly mailed to wrong members, according to a WTSP report.

A machine processing member statements was programmed to insert two premium statements per envelope instead of just one statement per envelope.

The premium statements included names, addresses and CarePlus identification numbers. No Social Security numbers were disclosed, according to the report.
