Epic rolls out ‘Healthy Planet’ module to Novant Health

Winston-Salem, N.C.-based Novant Health implemented Epic’s Healthy Planet, an analytics tool that provides clinicians with data related to patient health trends.

The population health module offers clinicians a dashboard of information to coordinate care delivery, monitor quality and engage patients. The tool enables clinicians to track performance on 15 utilization and preventative care metrics, including high blood pressure control, influenza immunization and breast cancer screening.

Healthy Planet provides clinicians with retrospective, real-time and predictive analytics, along with risk scoring tools and chronic disease registries to inform patient care. For example, the tool gives healthcare providers access to patients’ historical prescription patterns and frequency of prior hospitalizations.

“Over their lifetime, most people will see multiple providers, undergo a variety of screenings and experience health emergencies,” said Dave Garrett, CIO of Novant Health. “It is important that healthcare providers within the Novant Health system and those practicing outside of our system have access to this valuable patient information so they can provide appropriate preventive care.”
