The annual award recognizes a collaborative effort between a CIO who is also a CHIME member and a CHIME Foundation firm who jointly offer a benefit to the health IT community.
Encompass Health and Cerner were chosen for the award for their initiative to identify patients who are at risk of an unplanned acute-care transfer. The two organizations created a tool that provides early warning to clinicians and establishes timely intervention to reduce the likelihood of an unplanned transfer.
Encompass Health comprises a network of 133 hospitals, 245 home health locations and 82 hospice locations across 37 states.
