Edison, N.J.-based Hackensack Meridian Health has expanded access to an EMR dashboard that supports geriatric care across its acute and ambulatory care practices.
Developed by the Division of Geriatric Medicine at Hackensack University Medical Center in collaboration with the system’s Geriatric Quality Council, the dashboard is designed to improve outcomes for older adults by aligning with the Age-Friendly Health System 4M framework: medications, mentation, mobility and what matters.
The tool includes an automated prescription management feature called the geriatric prescription context, which checks for proper dosages and suggests safer alternatives, according to a January news release from the health system. A delirium predictive index is set to launch early in 2026, with a fall risk index also in development.
Leaders said the systemwide dashboard enables shared learning between acute and ambulatory providers to standardize and improve care for older patients.
