On January 11, Southwest Health learned of the data breach that exposed personal information including names, date of birth, Social Security numbers, driver’s license numbers, financial information and health insurance information, according to a July 5 press release.
Southwest Health notified anyone they believed was affected by the data breach, though there is no indication that any of the information has been misused.
