Dayton, Ohio-based Premier Health Partners is alerting patients to a data breach that exposed sensitive personal and health information.

The breach stemmed from unauthorized access to the system’s network between June 7 and July 12, 2023. Premier Health discovered suspicious activity in July 2023 and launched an investigation to determine the scope of the incident, according to a July 18 news release.

A review, which was recently completed, found that files accessed by an outside party may have included names, dates of birth, Social Security numbers, financial details, medical and insurance records, and various forms of government-issued identification.

Premier Health said there is no indication the compromised data has been misused.

Although operations were not disrupted and care continues as usual, the organization is notifying individuals whose information may have been involved. Those with verified contact information will receive written notifications and complimentary credit monitoring, according to the release.
