At least 20 healthcare organizations have reported data breaches affecting 100,000 or more individuals to federal regulators so far in 2026, according to the HHS Office for Civil Rights breach portal.
The 2026 breaches are already approaching last year’s totals. The 10 largest healthcare data breaches reported to HHS in 2025 affected more than 25 million people combined, Becker’s reported in December. The 10 largest breaches reported so far in 2026 have affected nearly 16 million people, with more than half the year remaining.
Here are the 20 largest healthcare data breaches reported to HHS so far in 2026:

- Illinois Department of Human Services: 705,017 people affected.
- ApolloMD Business Services (Ga.): 626,540 people affected.
- Erie Family Health Centers in Chicago: 570,000 people affected.
- Minnesota Department of Human Services: 303,965 people affected.
- North Texas Behavioral Health Authority: 285,086 people affected.
- Florida Physician Specialists: 276,498 people affected.
- Southern Illinois Dermatology: 160,312 people affected.
