Through the acquisition, Comvest Partners, along with co-investor Mosaic Health Solutions, will provide chronic case management services, nurse advisory services and a payer-facing platform to manage utilization, disease and case management services.
AxisPoint Health will be headquartered in Westminster, Colo.
