The intruders allegedly initially accessed the server on Oct. 9, 2012. The clinic did not learn of the breach until June 9, 2014, according to the hospital’s breach notification. The server has since been isolated.
Compromised data may include names, birth dates, Social Security numbers, addresses and phone numbers from some written imaging and radiology reports, though the hospital does not have any indication any personal information was viewed or copied to an unauthorized location, according to the notification.
