Carolinas HealthCare System taps Cerner’s population health platform

Charlotte, N.C.-based Carolinas HealthCare System selected Cerner’s HealtheIntent population health management platform. The health system, which already uses the Cerner Millennium EHR, will expand its remote hosting relationship with the health IT vendor.

The population health management platform pulls together data from multiple sources and can be used to coordinate and manage care for each individual patient. The platform includes:

●    HealtheEDW: An enterprise data warehouse solution
●    HealtheRegistries: A registries and scorecards solution
●    HealtheCare: A community care management solution
