The partnership aims to help ACOs, physician groups and independent practices gain insights into patient populations, coordinate care, control utilization and improve clinical outcomes while reducing costs, according to a news release.
It will focus on CareCloud’s application platform, and enable medical group clients to analyze data and identify at-risk patients and those in need of proactive intervention across multiple providers and facilities, the release states.
