Common Formats refer to the common definitions and reporting formats that allow healthcare providers to collect and submit information regarding patient safety events. AHRQ’s Common Formats Version 1.1 includes events descriptions, report and summaries, a user’s guide and technical specifications for electronic data collection and reporting.

The newly revised Common Format, Device or Medical/Surgical Supply, including HIT Device, will be incorporated into the next Common Formats Version 1.2.  The Common Format is available as a beta version on AHRQ’s website.
