Adventist Health launches home-based healthcare program for at-risk communities

Roseville, Calif.-based Adventist Health has partnered with in-home primary care provider Emcara Health to expand in-home care to high-risk communities in parts of California. 

Emcara Health will deploy care teams to meet patients at home, in a homeless encampment or in the emergency department to conduct a social determinants of health assessment, according to a July 11 press release. Emcara Health nurses and social workers will then connect and coordinate preventive care and clinical treatment through Adventist Health. 

"By working together with California residents who are most in need, this partnership has the ability to serve as a model for managed Medicaid plans and risk-bearing provider groups looking to improve health outcomes for vulnerable populations around the country," said Eric Galvin, CEO of Emcara Health. 

The partnership aims to address social barriers that influence a patient's health and to close care gaps for patients with complex care needs.
