Santa Ynez Valley Medical Associates in Solvang, Calif., and Frischer Medical Group in Downey, Calif., selected the CCM Program, too.
Additionally, the Londonberry, N.H.-based Southern New Hampshire Primary Care IPA chose Allscripts’ Value-based Care team to help them apply to become an ACO.
