The solution will connect ambulatory EHRs, inpatient systems and payer claim feeds to provide organizations with a comprehensive look into a patient’s care across all settings. The solution’s patient relationship management tool will also enable the ACOs to deliver appointment reminders and preventative care measures.
The ACOs’ goal is to improve care coordination, patient engagement and cost management across their combined 90 providers and more than 17,000 beneficiaries.
