Three ACOs in Florida — Central Florida ACO, Next ACO of Nature Coast and Space Coast ACO — will deploy eClinicalWorks' population health management solution.
The solution will connect ambulatory EHRs, inpatient systems and payer claim feeds to provide organizations with a comprehensive look into a patient's care across all settings. The solution's patient relationship management tool will also enable the ACOs to deliver appointment reminders and preventative care measures.
The ACOs' goal is to improve care coordination, patient engagement and cost management across their combined 90 providers and more than 17,000 beneficiaries.
