The FDA has clarified its policies regarding GLP-1s, stating that pharmacies must stop compounding GLP-1 drugs once branded versions are removed from the agency’s shortage list, signaling a narrower pathway for producing the high-demand therapies as supply stabilizes.
Under federal law, compounding is permitted only during active shortages. In an April 1 update, the FDA said the exemption is temporary and tied directly to supply conditions, warning that continued production of “essentially copies” outside those parameters could trigger enforcement action.
The move comes as the agency ramps up scrutiny of the compounded GLP-1 market, which expanded rapidly during recent shortages driven by demand for diabetes and weight loss treatments. In March, the FDA issued 30 warning letters to telehealth companies for misleading claims about compounded GLP-1 products, including suggestions that they were equivalent to FDA-approved drugs and failure to disclose the role of compounders.
The agency said compounded drugs are not FDA-approved and are not reviewed for safety, effectiveness or quality before being marketed. The warning letters are part of a broader crackdown launched in fall 2025, during which the FDA sent more marketing violation notices than in the previous decade.
