Granite Healthcare Network hospitals include Concord (New Hampshire) Hospital, Elliot Health System in Manchester, LRGHealthcare in Laconia, Southern New Hampshire Health System in Nashua and Wentworth-Douglass Hospital in Dover.

Cigna’s new collaborative accountable care program covers more than 23,000 people covered by a Cigna health plan who receive care from more than 900 healthcare professionals participating in the Granite Healthcare Network.

Registered nurses will serve as clinical care coordinators in the commercial ACO and help patients with chronic conditions navigate their healthcare system. Cigna will provide the nurses patient-specific data for patients at risk for readmission, overdue on health screenings or missing prescription refills. The nurses will then contact patients to help them get follow-up care, in an effort to reduce readmission costs.

The Cigna-Granite Healthcare accountable care model is believed to be the largest commercial ACO in New Hampshire.
