Medicaid Advantage population to get value-based care in Va.

Under a value-based care initiative, Virginia Care Partners has signed Transcend to provide population health management services to more than 10,000 Richmond, Va.-area Medicare Advantage patients.

VCP is a physician-led, value-based care network with more than 90 physician members. Under the agreement, Transcend will work with VCP physicians to track, analyze and manage Medicare Advantage patients' health.

VCP currently manages network contracts with HCA Virginia, a six-hospital network, as well as major insurers such as Humana, Aetna, Coventry, Optima, Cigna and UnitedHealthcare.

"When high quality physicians are partnered with a comprehensive population healthcare model, we have the opportunity to excel," said Transcend President Patrick Adams.
