How CHS, HCA, Tenet, and UHS’ CEO-to-worker pay ratios ranked in 2025

CEO-to-worker pay ratios varied widely across major for-profit health systems in 2025. Nashville, Tenn.-based HCA Healthcare reported the largest gap, followed by King of Prussia, Pa.-based Universal Health Services and Franklin, Tenn.-based Community Health Systems, according to their most recent proxy statements filed with the Securities and Exchange Commission.  

Dallas-based Tenet’s CEO, Saum Sutaria, MD, received $43,108,969 in total compensation for 2025, while the annual total compensation of Tenet’s median-compensated employee was $60,657, a 711-to-1 ratio. Tenet’s CEO-to-worker pay ratio was 406-to-1 in 2024, when Dr. Sutaria’s total compensation was $24,661,553 and the system’s median-compensated employee earned $60,698.

HCA Healthcare CEO Sam Hazen’s total compensation was $26,456,606 in 2025, which represented 420 times the pay of the system’s median employee, who earned $62,955. The ratio increased from 391-to-1 in 2024 and 356-to-1 in 2023, continuing an upward trend. HCA, which employed more than 316,000 workers in 2025, said the figure was a “reasonable estimate calculated in a manner consistent with SEC rules,” based on its payroll and employment records.

UHS came next with its CEO, Marc Miller, earning $16,148,937 in 2025, or 283 times the system’s median employee pay of $57,048. That ratio was consistent with 2024, when UHS reported a CEO-to-worker pay ratio of 281-to-1.

Lastly, CHS’ newly appointed CEO, Kevin Hammons, earned $4,772,869 in total compensation in 2025, 60 times more than the system’s median employee, who earned $79,925. The ratio highlighted a decrease from 2024, when former CEO Tim Hingtgen’s compensation represented a 79-to-1 gap. Mr. Hammons stepped into the permanent position in late 2025 after serving as president and CFO of the system, succeeding Mr. Hingtgen, who retired in September.

The disclosures are part of an SEC requirement for public companies to report the ratio of CEO compensation to median employee pay annually.
