Hospital-insurer network threats cluster in a ‘Goldilocks zone’: Study

Public threats by hospitals or insurers to end network contracts are concentrated in markets where neither side dominates, according to a study published Sept. 23 in Health Affairs.

The study, by researchers at Brown University in Providence, R.I., examined 14,918 relationships between 3,772 hospitals and 92 in-market commercial insurers for contracts ending between August 2021 and July 2025. Researchers cataloged public threats using a news database and manual review.

Researchers documented 1,249 episodes in which a hospital or insurer publicly announced or threatened network withdrawal, about 8% of relationships. Of those, 348, or 28%, ended with the hospital actually leaving the network, corresponding to about 2% of all relationships studied. Most episodes occurred after August 2023.

The rate climbed to about 16% where the hospital system controlled roughly 23% to 45% of local beds and the insurer held 30% to 44% of commercial lives. “There is a ‘Goldilocks zone’ where hospitals and insurers assess that the anticipated advantages of brinkmanship trump anticipated disadvantages,” the authors wrote, noting a party may be less able to credibly threaten withdrawal when its counterpart controls a very high share of beds or lives.

Negotiations involving for-profit hospitals were nearly twice as likely to involve public threats as those with nonprofit hospitals, though for-profit hospitals rarely left networks. National insurers made public threats in about 10% of their hospital relationships, more often than regional or single-state insurers, and hospitals commanding higher commercial prices were more likely to go public.

The authors said policymakers should assess both whether aggressive negotiations control costs and how they affect patients’ access, a calculus health system executives have described to Becker’s as difficult to predict.
