Under the program, which is run by HHS‘ innovation lab, organizations such as community agencies on aging receive federal reimbursement to reduce readmissions.
But only four out of 48 CCTP sites studied saw a significant reduction in readmission rates compared to the 2010 baseline rate, according to the early evaluation, which was produced by consulting firm Econometrica.
However, the early findings were based on evaluation activities underway during the initial project year, so it is not an indicator of overall program success, according to the report.
