Greensboro, N.C.-based Cone Health is sunsetting its Hospital at Home program, according to an Oct. 14 statement from the health system shared with Becker’s.

Cone Health made the decision following a comprehensive review of its priorities and the model’s sustainability.

The statement noted that health systems across the U.S. continue to evaluate hospital-at-home programs as they navigate evolving reimbursement models, operational complexity and long-term sustainability.

The program, which launched in April 2025, reported strong first-year results on June 1, including no patient deaths or healthcare-acquired infections. Cone Health had limited the program to six patients at a time and had planned to expand it once more payers covered the model.

“We are proud of the care provided through the program and grateful for the team members who made it possible,” the statement said. “A transition plan is being developed to guide a thoughtful wind-down during the fourth quarter, with a continued focus on supporting the small number of patients currently in this program, redeploying team members and ensuring continuity of care.”
