This is the second round of grants under the Transforming Clinical Practice Initiative — one of the largest federal investments uniquely designed to support clinician practices through nationwide, collaborative and peer-based learning networks that facilitate practice transformation. Support and Alignment Networks 2.0 will help clinicians prepare for the new Quality Payment Program, which CMS is implementing as part of the Medicare Access and CHIP Reauthorization Act.
Healthcare delivery systems and plans that provide quality improvement support to a large number of clinicians are eligible to apply for the grants. Grant recipients will provide tailored technical assistance to help clinicians and practices improve patient outcomes and reduce Medicare, Medicaid and Children’s Health Insurance Program expenditures.
Letters of intent to apply are due July 1 and applications are due July 11.
