Participants in the model are based in eight states: Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina and Washington.

The program, which runs through 2034, aims to improve care management and coordination, help primary care providers partner with specialists, and use community-based connections to address patients’ health needs and health-related social needs, such as housing and nutrition.

The 10.5-year multi-payer model has three participation tracks that build upon previous primary care models, such as the comprehensive primary care, CPC+, and primary care first models, as well as the Maryland primary care program.
