Here are five things to know.
1. The program, which launches this month, will focus on delivering care to people with severe mental illness in Cook County, who have been assigned to court-ordered behavioral health therapy, according to a news release.
2. CCHHS said the program will ultimately enhance and coordinate services available to patients from various local and state partners.
3. A majority of assisted outpatient treatment candidates include patients at John J. Madden Mental Health Center, and Chicago-Read Mental Health Center, and other individuals who have a history of frequent hospitalizations, interactions with the criminal justice system or both, CCHHS said.
4. Commenting on the grant, Dr. Michael Colombatto, director of ambulatory behavioral health at CCHHS, said in the release, “The goal of this project is to develop infrastructure in Cook County to ensure continuity of care and adherence to treatment for the most severely mentally ill. We hope to demonstrate that effectively linking people to the resources they need to be successful in treatment can significantly reduce recidivism in inpatient facilities, emergency departments and correctional facilities.”
5. According to CCHHS, a number of agencies are collaborating on the grant. These agencies include Cook County States Attorney’s Office, the Illinois Guardianship & Advocacy Commission, Haymarket Center, Community Counseling Centers of Chicago, Metropolitan Family Services, Human Resources Development Institute, Habilitative Systems, South Suburban Council on Alcoholism and Substance Abuse and Family Guidance Centers, as well as a steering committee of participating organizations and patient advocates.
