A Healthcare Cost and Utilization Project Statistical Brief, released by the Agency for Healthcare Research and Quality, found hospital readmission costs exceeded initial admission costs for about two-thirds of common diagnoses listed in ICD-10's diagnosis coding system in 2016. 

5. CMS seeks ideas to reduce administrative burden on physicians  
CMS is asking patients, families, providers and other healthcare stakeholders on prior authorization, reporting, documentation and coding, along with policies and requirements for rural residents and people dually enrolled in Medicare and Medicaid.
