The annual out-of-pocket cap for individuals includes copayments, coinsurance fees and deductibles, but the study found 38 percent of the 9,752 health plans studied excluded deductibles from their out-of-pocket expense calculations, including Blue Cross Blue Shield insurers in Illinois, Kansas and Texas.
