In her new, two-year role, Ms. Romano will work to incorporate social determinants of health in a patient’s EHR, according to an Aug. 11 Premise Health news release. Social determinants of health are the socioeconomic factors in a person’s life that affect their health outcomes.
“Incorporating social determinants of health into clinical practice is an important part of delivering comprehensive, whole-person primary care that improves well-being,” said Jon Leizman, MD, chief medical officer for Premise, said in the release. “Empowering our providers to have conversations about these factors at the point of care can ultimately lead to better outcomes, improve member satisfaction, enhance clinical quality, and drive cost-savings.”
