The health system is implementing Jvion’s cognitive machine software, which identifies at-risk patients based on medical history and social determinants of health information, such as lack of access to transportation or housing insecurity. The AI software also recommends steps to mitigate a patient’s risk of a readmission.
Northwell Health will focus on readmissions and avoidable admissions within its managed care population during the initial implementation phase.
