How Kaiser uses Epic for at-home care

Oakland, Calif.-based Kaiser Permanente’s hospital at home program uses three different digital health platforms to manage patient care, document results and communicate with patients.

The health system uses Epic’s ambulatory module to manage the program. Patients receive wireless vital sign equipment that uploads their results to Epic, according to a December study published in The American Journal of Managed Care.

Beyond Epic, the health system deploys Cesia Continuum, software developed by Medically Home Group, to gather clinical information.
