The group, which comprises leaders from more than 40 health systems, meets regularly to discuss their experiences solving challenges associated with social determinants of health as well as removing barriers to care in their communities. Members of the collaborative include officials from Kansas City, Mo.-based Truman Medical Centers, Los Angeles County Department of Health Services and Marshall University School of Medicine in Huntington, W.Va.
Cerner has built screening tools and questionnaires into its EHR to help health systems identify challenges their patients may face related to social determinants of health.
