CareInMotion, Allscripts’ population health management platform, aims to support providers as they develop care plans that span the care continuum, including post-acute options. The solution delivers care coordination, patient engagement and data analytics capabilities targeted toward managing care transitions.
As part of the agreement, Catholic Medical Center will also purchase Allscripts Care Management, Allscripts Care Director, CarePort Guide, CarePort Connect and CarePort Insight.
